To access the online shopping list, you must have Adobe Reader installed.
Click here to download the free Adobe Reader.
To submit order online:

Click here to open the shopping list
- Fill in the requested agency information
- Fill in the quantity of each item you'd like to order
- Click the Submit button
- In the email that opens, type Order
in the subject line and send email
If your email will not allow the action or attaches the pdf as a different file format (such as .dat), use the following method.
To send completed shopping list as an email attachment:
- Click here to open the shopping list
- Fill in the requested agency information
- Fill in the quantity of each item you'd like to order
- Select File > Save As... to save completed form as
your agency nameorder.pdf on your computer
- Select Insert > File... and browse to the saved
shopping list to attach to email
- Send the email
If you have trouble viewing or filling out the pdf form, use the following method.
To print the shopping list to fax or phone in:

Click here to open the shopping list
- In the shopping list browser window,
click File > Print on the main menu
- Orders may be faxed to 605-335-6617 or
phoned in to 605-335-0364
